Looking to apply?

If you are looking to apply, either for yourself or on behalf of someone else, then a referral application form must be completed.

If you are completing a self-referral and/or don’t have a risk-assessment or CPA, then please call the Sustain and we will assist you with the process.

Applying for someone else?

If you are applying on behalf of someone and are from an agency e.g Hospital, CPS, then please complete the Referral Application

If applicable, please send over details of the Care Programme Approach (which would be provided by the community mental health team,) or a Risk Assessment.

The Next Steps

Once these are completed we then have an interview with the individual or their care psychiatric nurse, family member or friends. If the individual and the support staff are happy with the interview a move in date will be set.

If the individual is not successful they will be informed and will be entitled to make an appeal.

We are limited with the number of properties adapted for wheelchair users, however, there are some provisions in our portfolio.
